Please complete the following information to request a Cookie Booth. Items marked with a red asterisk (*) are required fields. The form can not be processed without this information. For Brownie and Junior troops, a maximum of 1 booth booth per 3 registered sellers will be awarded from the selections below (eg: if you have 9 registered sellers, you will be awarded a maximum of 3 booths). For Cadette, Senior and Ambassador troops, a maximum of 1 booth booth per 2 registered sellers will be awarded from the selections below (eg: if you have 8 registered sellers, you will be awarded a maximum of 4 booths). Submissions are only accepted between 10:00 AM and 8:00 PM on Mondays and Thursdays during the Cookie Sale.
